Adaptations and Better Care Fund

Trish is an experienced project manager across multi-partner projects reviewing and developing adaptations and disabled facilities grant services to meet the broader aims of the Care Act and the Better Care Fund. Helping people maintain independent living whether in older age or due to a disability can be achieved through ensuring they are living in suitable accommodation. Timely planning and making positive choices at an early stage can avoid having to make decisions is a crisis.

A recent project looking at adaptations services in relation to disabled facilities grants within a framework of the Better Care Fund and integration has enabled new housing options specialist posts to be adopted within shared health and social care teams in order to ensure housing advice and options information is available across age groups at an early stage.  Developing partnerships across sectors is key to developing new integrated pathways across health, social care and housing and engagement with both statutory and voluntary sector partners at all levels will ensure this is achieved.
